Job summary

Swan Hospice is seeking a Certified Home Health Aide (CHHA) with hospice experience to join our compassionate team. We deliver quality hospice care to patients in homes, facilities, and community settings, emphasizing dignity and respect for residents and families.

Ideal candidates bring an NJ HHA license and at least one year of hospice experience, plus dependable transportation to reach patients across our service areas.

Qualifications

• HHA license in New Jersey
• Minimum of one year experience in hospice care
• Dependable transportation to patient locations

Responsibilities

• Assist with prescribed exercises and ROM as taught by Hospice Nurse.
• Assist with toileting, positioning and transfers; frequent turning and positioning of bed-bound residents.
• Assist with ADLs including personal hygiene, grooming, mouth care, skin care and nails.
• Assist or remind patient to take medications per regulatory guidelines.
• Assist with light housekeeping of the patient’s unit and laundry.
• Feed and assist with meals.
• Take temperature, pulse and respiration as supervised by the Hospice Nurse.
• Report changes to the Hospice Nurse and document communications.
