Job summary

Pathways Healthcare, Llc is seeking a Hospice Home Health Aide to support patients in Worcester and surrounding areas. The role offers a Monday–Friday schedule from 8:30 AM to 5:00 PM and an hourly rate of $25.00.

Qualified CNAs are preferred in Massachusetts; Home Health Aide certification is accepted. Candidates must be 18+, with CPR, a valid driver's license, and the ability to follow care plans and document services provided.

Qualifications

Must be 18 years of age or older.
CNA preferred; Home Health Aide certification accepted.
Ability to read and follow written care plans and document services provided.
Ability to work independently with minimal supervision.
Current CPR certification required.
Strong interpersonal skills and professionalism.
Valid driver’s license and reliable insured vehicle.
Ability to assist patients with repositioning, transferring, and other physical tasks.

Responsibilities

Provide personal care assistance such as bathing, grooming, dressing, and mobility support.
Observe and report changes in patients’ condition to the supervising nurse or therapist.
Document care provided and patient observations accurately.
Follow the care plan established by the Registered Nurse.
Maintain a safe and supportive environment for patients in their homes.
Promote quality, continuity, and safety of patient care.
