Job summary

Pathways Healthcare seeks a compassionate Hospice Home Health Aide to support patients in Worcester and surrounding areas. Part-time position with a Monday–Friday schedule from 8:30 AM to 5:00 PM and an hourly rate of $25.00. CNA in Massachusetts preferred; Home Health Aide certification accepted.

Must be at least 18, able to follow care plans, and have CPR certification. Eligible candidates will document care accurately, work independently with minimal supervision, and provide essential

Qualifications

Certified Nursing Assistant (CNA) in Massachusetts preferred.
Home Health Aide certification accepted.
Must be 18 years of age or older.
Current CPR certification required.

Responsibilities

Provide personal care assistance such as bathing, grooming, dressing, and mobility support.
Observe and report changes in patients’ condition to the supervising nurse or therapist.
Document care provided and patient observations accurately.
Follow the care plan established by the Registered Nurse.
Maintain a safe and supportive environment for patients in their homes.
Promote quality, continuity, and safety of patient care.
