Job summary

Samaritan is seeking a Home Health Aide in New Jersey to provide compassionate, patient-centered care in private homes and facilities. This full-time position requires travel within the service area, a valid NJ CHHA certification, and a commitment to delivering high-quality hospice care.

You will implement care plans under a registered nurse, assist with daily activities, and document care in electronic systems.

Qualifications

• NJ CHHA certification required.
• Driver's license and willingness to travel throughout the service area.
• Excellent verbal and written communication; bilingual is a plus.

Responsibilities

• Caring for hospice patients in private homes and/or healthcare facilities.
• Assisting with activities of daily living, such as personal hygiene, meals, ambulation, repositioning.
• Implementing plan of care under the directive of a registered nurse.
• Performing light housekeeping in patient area only, when needed.
• Documenting patient care in electronic software.
• Ability to travel to patient homes and facilities as assigned.
• Represent Samaritan in a positive manner to patients, families and the community.
• Other duties as assigned.
