Job summary

Baystate Health is seeking a Home Health Aide (HHA) to join its Hospice team in western Massachusetts. The role focuses on compassionate, patient-centered end-of-life care in home and community settings, working with RNs and interdisciplinary teams to support dignity and quality of life.

The HHA will assist with ADLs, mobility, and comfort measures, while documenting care and collaborating with the hospice team to ensure safety and continuity of care. Employment is part-time with day shifts.

Qualifications

• Must have Home Health Aide (HHA) or CNA certification.
• Valid Driver’s License required.
• GED or HiSET required; BLS CPR certification preferred.

Responsibilities

• Provide compassionate, patient-centered personal care to hospice patients in homes and community settings.
• Assist with activities of daily living while preserving privacy and dignity.
• Support safe mobility, transfers, and ambulation; promote safety and independence.
• Observe and report changes in patient condition to supervising RN.
• Offer emotional support and companionship to patients and families.
