Job summary

Baystate Health is seeking a Home Health Aide (HHA) to join our Hospice Team in Springfield, MA. The role focuses on compassionate, in-home end-of-life care, working with an interdisciplinary team to support patients and families.

You will assist with ADLs, mobility, and personal care, while documenting care and reporting changes to the RN. The position offers day shifts, 40 hours per week, and competitive benefits within a large regional health system.

Qualifications

• Home Health Aide or CNA Certification is required.
• Valid Driver’s License is required.
• GED or HiSET (Required) / High School Diploma may be accepted as alternative qualifications.

Responsibilities

• Provide compassionate, patient-centered personal care to hospice patients in their homes and community settings.
• Assist with activities of daily living (ADLs) including bathing, grooming, dressing, toileting, and oral hygiene.
• Assist with safe mobility, transfers, positioning, and ambulation.
• Observe and report changes to supervising RN promptly.
• Provide emotional support and companionship to patients and families.
• Maintain a clean, safe patient environment and assist with light housekeeping related to care.
• Follow care plans, infection prevention protocols, and hospice policies.
• Communicate patient needs to the hospice team and document care provided.
