Job summary

Baystate Health is seeking a Home Health Aide to join our Hospice Team for care in patients' homes and community settings. Work closely with RNs, physicians, and interdisciplinary teams to provide compassionate, dignity-driven end-of-life care that supports patients and families.

The role emphasizes personal care, ADLs assistance, emotional support, and collaboration within a skilled hospice environment, with a schedule of full time days and a 40-hour week across Western Massachusetts.

Qualifications

• Home Health Aide or CNA Certification.
• Valid Driver's License.
• HGED or High School Diploma.

Responsibilities

• Provide compassionate, patient-centered personal care to hospice patients in their homes and community settings.
• Assist with activities of daily living, including bathing, grooming, dressing, toileting, and oral hygiene.
• Assist with safe mobility, transfers, positioning, and ambulation.
• Observe and report changes in patients’ condition to the supervising RN.
• Provide emotional support and companionship to patients and families.
• Maintain a clean, safe patient environment and assist with light housekeeping related to patient care.
• Follow care plans, infection prevention protocols, and hospice policies under supervision.
• Collaborate with the hospice interdisciplinary team and document care provided.
