EvergreenHealth Hospice is seeking an experienced RN to provide total direct nursing care for patients within established standards and policies. The role emphasizes coordination with families, physician partners, and the interdisciplinary team to ensure high-quality end-of-life care.

Qualifications

• Graduate of a nursing program (Diploma or ADN) required.
• Licensed as a Registered Nurse in the State of Washington.
• Current Healthcare Provider BLS certification by date of hire.
• Current Washington State Driver's license and proof of insurability.

Responsibilities

• Provides direct patient care.
• Assesses nursing needs of assigned patients (including home health).
• Coordinates education for patient and family.
• Initiates start of care through admission process.
• Plans, implements, and evaluates patient care with family involvement.
• Collaborates with physicians and inter-disciplinary team to develop care plans.
