Job summary

Home Care Select Hospice is seeking a dedicated Registered Nurse to provide hospice care in patients' homes and community settings within the Greater Houston area. The role emphasizes comprehensive assessment, individualized care plans, and skilled nursing interventions to manage pain and symptoms.

The RN will educate families, coordinate with an interdisciplinary team, and ensure regulatory compliance while delivering compassionate, patient-centered care in a home-based environment.

Qualifications

• RN license in Texas with hospice or palliative care experience.
• Experience providing bedside nursing care, including pain and symptom management.
• Excellent communication with patients, families and the interdisciplinary team.
• Strong organizational and documentation skills; able to manage multiple patients.

Responsibilities

• Assess patient conditions and develop individualized care plans.
• Deliver skilled nursing interventions, including medications and symptom control.
• Monitor vitals and document clinical findings accurately.
• Coordinate with physicians, social workers, chaplains, and other team members for integrated care.
• Educate patients and families on disease processes and comfort measures.
• participate in care conferences and quality improvement activities.
