Job summary

UPMC Family Hospice is hiring a Home Care Aide for the Wellsboro area, covering Tioga, Bradford, and Potter. The role supports patients in their homes Monday through Friday, 8:00 am–4:30 pm, with periodic weekend rotation and two holidays per year.

Responsibilities include direct patient care, accurate documentation, and collaboration with the home health team. Preferred candidates have a High School Diploma or GED, at least 1 year of relevant experience, and Nurse Aide training (75 hours).

Qualifications

• High School Diploma or GED is required.
• 75 hours of Nurse Aide training required; if none, training may be provided on hire.
• BLS/CPR certification preferred or required.
• Driver's license required.
• Nurse Aide certification preferred; 1 year of experience preferred.

Responsibilities

• Provide designated home care under supervision to achieve identified goals.
• Maintain annual competency evaluations as required by regulations.
• Participate in compliance training and adhere to policies.
• Document accurately and promptly in patient records.
• Communicate changes in patient condition promptly to the care team.
• Engage in care planning and process improvement to enhance service quality.
• Attend all required agency meetings.

UPMC Family Hospice is hiring a Home Care Aide for the Wellsboro area, covering Tioga, Bradford, and Potter.

• Monday-Friday 8:00 am-4:30 pm
• Two holidays a year
• Weekend Rotation (Saturdays approx. every 4th/5th weekend)

Purpose

Provide designated care under the direction of the primary service provider in the home in order to achieve identified goals. Function as an extension of skilled nursing services and/or rehabilitation services in the home.

Responsibilities

• Must maintain annual competency evaluation scores as determined by regulatory requirements
• Attends compliance training and adheres to the organization's standards of conduct, policies, and procedures.
• Proven ability to facilitate change.
• Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
• Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
• Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization.
• Participates in all required agency meetings.
• Communicates appropriately with the interdisciplinary home care team promptly, reporting any changes in the patient’s condition to appropriate professional staff.
• Maintains and completes documentation accurately and promptly while in the patient's home and in accordance with agency and regulatory standards.
• Provides safe and direct personal care to patients in the home setting under direct supervision of an appropriate professional.
• High School Diploma or GED - Required
• Hands-on direct patient personal care in the last 5 years preferred
• One (1) year of experience - Preferred
• Certified Nurse Aide – Preferred
• Minimum of 75 hours of Nurse Aide training – Required (If no previous nurse aide training, applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire.)
