Job summary

Lorian Health in San Diego is seeking a Hospice Specialist to join our patient-centered home care team. You will build trusted relationships with healthcare professionals and referral sources to expand access to hospice services.

In this role, you will educate partners, manage marketing accounts, coordinate referrals, meet with patients and families to provide support, and advocate for patients across the hospice team and community.

Qualifications

• 2+ years of healthcare sales or marketing experience.
• Hospice or palliative care marketing experience strongly preferred.
• Proven ability to build and maintain relationships with healthcare professionals, patients, families, and community organizations.
• Strong communication, critical thinking, judgment, and relationship-building skills.
• Experience with crisis intervention and family support.
• High school diploma or GED required; bachelor's degree in a related field preferred.

Responsibilities

• Build and maintain relationships with healthcare professionals and community referral sources.
• Educate referral partners and the community about hospice services and the hospice benefit.
• Manage marketing accounts and develop follow-up and continuing education plans.
• Coordinate and follow up on pending referrals and communicate referral outcomes.
• Meet with patients and families to provide support and assist with hospice enrollment.
• Serve as a patient advocate across the hospice team, program, and community.
• Participate in staff meetings, interdisciplinary group meetings, training, and in-service education.
• Plan and participate in engaging marketing initiatives, networking events, educational lunches, conferences and community activities that strengthen relationships with case management and healthcare partners while increasing Lorian's visibility in the community
