Job summary

Choice Hospice, Inc. is seeking a Hospice Field Registered Nurse (RN) PRN for our Enid, OK location.

The RN will deliver compassionate, patient-centered care to individuals facing life-limiting illnesses in home and community settings, coordinating with physicians, social workers, and chaplains to ensure comfort and dignity. Responsibilities include initial and ongoing assessments, developing and updating the plan of care, educating patients and families about end-of-life care, and providing

Qualifications

Active RN license in good standing.
CPR certification required.
Valid driver's license and auto insurance.
Minimum of 2 years hospice RN experience preferred.

Responsibilities

Initial and ongoing comprehensive assessments of the patient’s physical, functional, psychosocial, and environmental needs.
Develop and update the plan of care and ensure continuity of care with IDG input.
Educate patient and family about disease process, self-care, end-of-life care, nutrition and dietary needs; provide staff training as needed.
Determine scope and frequency of services based on acuity and family needs; assess caregiver capacity.
Provide clinical direction to Hospice Aide and LPN/LVN to ensure quality care.
Prepare progress notes documenting progress toward goals and maintain accurate documentation.
