Job summary

Remarkable Hospice is seeking a Hospice Education Specialist to expand reach by developing referral sources, educating the community, and representing the company with professionalism and compassion.

You will build partnerships with physicians, hospitals, and facilities, conduct outreach, and support admissions while ensuring HIPAA-compliant documentation and strong cross-team collaboration.

Qualifications

• High school diploma or equivalent; Bachelor's degree preferred.
• 1–3 years in sales, marketing, or business development; home care or hospice preferred.
• Valid driver's license and auto liability insurance; compliant with regulations.

Responsibilities

• Build and maintain relationships with physicians, hospitals, SNFs, and referral partners.
• Conduct regular visits and calls to referral sources to promote home health services.
• Represent Remarkable Hospice at health fairs and community events.
• Deliver presentations to healthcare professionals and community leaders.
• Educate on eligibility, home health philosophy, and personalized care.
• Collaborate with intake and clinical teams for smooth transitions.
• Meet or exceed monthly referral and admissions goals.
• Document all contacts in the CRM in HIPAA-compliant manner.
