DIGNITY HOSPICE - DIRECTOR OF PATIENT CARE

Job Description

The Director of Patient Clinical Service is a registered professional nurse who ensures quality of patient care in the home. Primary function is for the overall administration of the clinical departments and monitoring of appropriate staffing and productivity. Coordinates care with the Interdisciplinary Group/Team, patient, family and referring agency. Monitors and analyzes QAPI issues and directs clinical staff with regard to the Hospice policies and procedures.

Key Responsibilities

  • Assumes responsibility for continuity, quality, and safety of services delivered in compliance with state and federal regulations.
  • Must be able to act as Alternate Administrator (meet the requirements set forth in Texas Administrative Code Title 26 rule §558.244 )
  • Promotes the philosophy and administrative policies of the Hospice, to ensure quality of patient care.
  • Participates in activities relevant to professional services including development of qualifications and assignment of the Hospice personnel
  • Ensures core services are available to respond to meet patient care needs at all times.
  • Supervises and provides direction to subordinates, in an effort to ensure quality, compliance with the Plan of Care, assessment and reassessment of patients’ needs, and continuity of services by appropriate health care personnel.
  • Monitors the QAPI Program and assures appropriate corrective measures are performed
  • Promotes compliance with all fiscal intermediary and/or other third- party payors, through education, coaching, and other assistance as necessary
  • Participates as a member of the Interdisciplinary Group/Team (IDG/T), assists in development of the IDG/T plan, and participates in QAPI activities.
  • Monitors systems and identifies problem areas to Administrator both verbally and through written reports.
  • Ensures mandatory continuing education requirements of the Hospice and licensing board are met
  • Promotes and educates regarding the concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.
  • Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
  • Flexibility to travel as needed for work-related assignments to Dallas, Fort Worth
