JourneyCare Hospice is seeking a Hospice LPN to join our growing Crisis Care team on a PRN basis! New competitive salaries and flexible hours!

HOURS: As Needed. Shifts will be 12-hour shifts from 7PM to 7AM, including weekdays and weekends.

TERRITORY: Schaumburg, Rolling Meadows, Palatine, IL, and surrounding areas.

SALARY: $26-$38 hourly, offer based on years of experience.

Qualifications:

• Graduate of an accredited practical nurse or vocational nursing program.
• At least three years of nursing experience; community health, hospice, or medical/surgical experience preferred.
• Currently licensed as an LPN/LVN in the practicing state.
• Must possess current CPR certification.
• Good verbal and written communication skills and organizational skills.
• Valid driver’s license with insured vehicle as per state requirements.
• Understanding of hospice philosophy and the needs of terminally ill patients.

What You’ll Do:

• Provide direct patient care as defined in the State Nurse Practice Act.
• Implement current nursing practices following comprehensive assessments and the plan of care.
• Accurately and timely document patient services to reflect the plan of care.
• Assess and educate patients and families/caregivers about diagnosis and self-care.
• Coordinate hospice services and report needs to the interdisciplinary team.
• Manage pain and symptoms, evaluate treatment responses.
