Emblem Home Health & Hospice in the Phoenix area seeks a Hospice Liaison to build relationships with physicians, hospitals, and referral sources, guiding families to compassionate hospice services. This role blends marketing with patient-centered care, emphasizing dignity and comfort.

You will educate healthcare professionals, document outreach, and collaborate with care coordination teams to improve access to hospice services.

Qualifications

• Bachelor’s degree in Marketing, Business Administration, or related field.
• Minimum 3 years of healthcare marketing experience, hospice experience strongly preferred.
• Strong communication, relationship-building, and public relations skills.
• Ability to work independently while maintaining a compassionate, patient-first approach.
• Demonstrated organizational skills and ability to meet outreach goals.
• Reliable transportation, AZ driver’s license, and proof of insurance.

Responsibilities

• Develop and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other referral sources
• Educate healthcare professionals and community groups about hospice services and benefits
• Serve as a knowledgeable, compassionate resource for patients and families during the referral and admission process
• Document outreach activities and maintain accurate records of interactions
• Ensure all messaging reflects our commitment to dignity, comfort, and quality care
• Collaborate with care coordination teams to support smooth transitions for patients
• Monitor community needs and identifies opportunities to improve access to hospice care
