Job summary

Alternate Solutions Health Network is seeking a Community Market Manager and Educator to direct growth and build strong relationships with agencies and referral sources within the assigned territory. The role focuses on developing market strategies, coordinating services, and ensuring growth targets are met.

Responsibilities include managing accounts, collaborating with leadership on annual plans, and serving as a liaison to improve patient care coordination and discharge planning.

Qualifications

• 3–5 years of market growth and maintenance experience, preferably in home care or in the medical field or a combination of relationship / clinical delivery.
• Ability to travel daily within assigned region.
• Valid driver’s license and auto insurance.
• Bachelor’s Degree or equivalent combination of education, training and experience.

Responsibilities

• Manage and oversee overall community growth and strategic plan for dedicated agencies while also managing daily assigned accounts.
• Work with region and agency leadership to implement growth and conversion strategies and prioritize key customers.
• Review patient insurances and medical documentation to determine home care appropriateness.
• Coordinate health care services as ordered by the attending physician.
• Assist hospital/facility personnel in the discharge planning process.
• Build and maintain lasting positive relationships with patients/clients, facility/hospital personnel, physicians, and any other team members.
• Establish and maintain relationships, arrange appointments, and deliver educational programs to keep referral sources current in local agency service offerings.
• Act as a liaison between referral sources and company staff to provide information on company services, resolve problems and maintain positive customer relations.

The Community Market Manager and Educator helps direct the activities of assigned community staff and provide relationship development and management related support to market/agency/program management to ensure market growth and profit targets are achieved or exceeded.

Manage and oversee overall community growth and strategic plan for dedicated agencies while also managing daily assigned accounts.

• Work with region and agency leadership to implement appropriate growth and conversion strategies and prioritize key customers in accordance with market strategic growth plan
• Reviews patient insurances and medical documentation to determine home care appropriateness
• Coordinates health care services as ordered by the attending physician
• Assists hospital/facility personnel in the discharge planning process
• Builds and maintains lasting positive relationships with patients/clients, facility/hospital personnel, physicians, and any other team members
• Notifies the referring facility manager before contacting patients
• Promotes well-being of patients as a part of the facility team(s)
• Work with leadership to develop and execute market-specific annual strategic plan/budget and prepare quarterly admissions updates to ensure achievement of established growth targets and compliance with established budgetary constraints
• Manage time and resources according to plan
• Maintain assigned territory and call on physicians, multiple facility types such as hospital, skilled nursing, rehabilitation, facility discharge planning services and/or management, assisted living and other senior living settings within assigned territory
• Maintain detailed territory list, prioritizing accounts in accordance with the market/territory growth plan. Maintain detailed territory management systems records as required
• Meet with market leadership and community team to discuss market growth activities, new opportunities, potential problems, etc.

• Comply with technology processes and expectations
• May have responsibilities as a mentor, as designated by VP Partnership and Community

MANAGEMENT RESPONSIBILITIES

• Responsible for day-to-day management of Community marketing personnel and activities to meet/exceed assigned admissions, growth, and conversion targets
• Interviewing, hiring, training and termination of department personnel
• Provides consistent coaching and provides challenge and opportunity to direct reports
• Performs coaching, counseling and discipline as needed
• Timecard management
