Job summary

Frontpoint Health in Dallas, TX seeks a Hospice Community Liaison who serves as the bridge between the Hospice and the community. The role focuses on developing and promoting hospice programs and services while upholding quality care standards and efficient use of resources.

Responsibilities include counseling the Hospice about community needs, program development with personnel and financial planning, policy development for new programs, and building relationships with physicians and community

Qualifications

• Graduate of an accredited college/university is preferred.
• Minimum 2 years of experience in the Hospice field (required).
• Excellent communication and interpersonal skills.
• Strong organizational and time management skills.

Responsibilities

• Provides counsel to the Hospice regarding needs of the community; program development including personnel needs and financial needs.
• Collaboratively develops and maintains policies specific to new programs.
• Demonstrates knowledge of and ensures compliance with all local, state, and federal laws relating to marketing of the Hospice.
• Assists in directing programs through collaboration with administrators and supervisory personnel.
• Partners with the Interdisciplinary Team to support patient/family care.
• Establishes a public relations program for interpretation of the Hospice’s services and to foster relations with physicians and community agencies.
• Carries out other duties as assigned by the Governing Body.
