Job summary

Frontpoint Health in Dallas, TX seeks a Hospice Community Liaison to serve as the primary link between the Hospice and the community, addressing community and customer needs while promoting hospice programs and services and upholding high standards of care.

You will collaborate with interdepartmental teams, support policy development for new programs, ensure compliance with laws, and help optimize patient/family care and resource utilization.

Qualifications

  • Graduate of an accredited college/university preferred.
  • Minimum 2 years of experience in the Hospice field.
  • Excellent communication and interpersonal skills.
  • Strong organizational and time management skills.

Responsibilities

  • Provides counsel to the Hospice regarding needs of the community; program development including personnel needs and financial needs.
  • Collaboratively develops and maintains policies specific to new programs.
  • Demonstrates an in-depth knowledge of, and ensures compliance with, all local, state, and federal laws relating to marketing of the Hospice.
  • Assists in direction of programs through collaboration with, and delegation of responsibility to, administrative and supervisory personnel.
  • Partners with the Interdisciplinary Group/Team to support safe and effective patient/family care.
  • Establishes a public relations program for interpretation of the Hospice's services and to foster good working relations with physicians and community agencies.
  • Carries out other duties as assigned by the Governing Body.
