Job summary

Del Corazon Hospice is seeking a compassionate Hospice Certified Nursing Assistant (CNA) to serve patients in Los Alamos, White Rock, and surrounding areas. The role involves direct patient care in SNFs, assisted living facilities, and homes, with daily travel within the service area.

The candidate should hold a current New Mexico CNA license, have reliable transportation, and demonstrate strong communication and teamwork in a hospice setting.

Qualifications

• Current New Mexico CNA license (required)
• Hospice or home health experience preferred
• Valid driver's license and reliable transportation
• Ability to travel within service area daily
• Strong communication and compassionate bedside manner
• Ability to work independently and as part of a team
• Must pass background check and drug screening

Responsibilities

• Provide personal care and emotional support to hospice patients in SNFs and private residences
• Assist with activities of daily living (bathing, grooming, toileting, transfers, feeding)
• Observe and report changes in patient condition to the clinical team
• Support families with education and comfort measures
• Maintain accurate documentation per hospice and regulatory guidelines
• Work collaboratively with nurses, social workers, chaplains, and the interdisciplinary team
• Maintain patient dignity, privacy, and comfort at all times
• Travel throughout the service area daily

Position Overview

We are seeking a Hospice Certified Nursing Assistant (CNA) to serve patients primarily in Los Alamos and White Rock, providing care in Skilled Nursing Facilities (SNFs), Assisted Livings and patients' homes. This role requires compassionate hands-on care, excellent communication, and a heart for hospice.

Key Responsibilities

• Provide personal care and emotional support to hospice patients in SNFs and private residences
