Join to apply for the Hospice CNA - (PRN) role at Traditions Health

Primary function is to provide personal healthcare and related services to the patient/client in his or her place of residence; to assist in providing a safe and clean environment, work cooperatively with the patient/client and family, and share observations and problems with the supervisor. Works under the direction and supervision of a registered nurse.

Job Qualifications
  1. Education: High School Diploma Preferred.
  2. Licensure: Must have a current driver's license; Must have a valid Certified Nursing Assistant/CNA license.
  3. Experience: At least one year experience preferred as a Home Health Aide or Nursing Assistant in a hospital, nursing home or home health agency. Must have completed a State-established or other training and competency evaluation program that meets State and Federal requirements, or a comparable program. Continuous employment in home health aide services for over 24 months since last completion is necessary to be considered current.
  4. Skills: Ability to read, write, and verbally report clinical information; interest in the welfare of the ill and elderly; proof of current CPR; successful completion of the Aide Competency Evaluation Skills Checklist and written Skills Test.
  5. Transportation: Reliable transportation and valid auto liability insurance.
Environmental and Working Conditions

Works in patients'/clients' homes under various conditions, possible exposure to blood, body fluids, and infectious diseases; flexible schedule; local travel; exposure to weather.

Physical and Mental Effort

Prolonged standing and walking; ability to lift up to 50 pounds; work under stressful conditions; manual dexterity; use of medical equipment at home.

Essential Functions
  1. Provides direct patient/client care under RN supervision and according to the Aide Plan of Care.
  2. Assists with safe transfers and ambulation.
  3. Reports changes and documents care provided.
  4. Practices infection control; maintains a clean and safe environment.
  5. Communicates respectfully with patients, families, and staff.
  6. Respects patient privacy and property.
  7. Assists with nutrition and fluid intake.
  8. Manages time effectively and contributes to agency operations.
  9. Performs treatments and procedures per policy.
  10. Attends 12 hours of in-services annually for professional growth.
  11. Supports agency philosophy and policies to ensure quality care.
