Join to apply for the Hospice CNA - (PRN) role at Traditions Health

Primary function is to provide personal healthcare and related services to the patient/client in his or her place of residence; to assist in providing a safe and clean environment, work cooperatively with the patient/client and family, and share observations and problems with the supervisor. Works under the direction and supervision of a registered nurse.

Job Qualifications
  1. Education: High School Diploma Preferred.
  2. Licensure: Must have a current driver's license; current Certified Nursing Assistant license/certification.
  3. Experience: At least one year experience preferred as a Home Health Aide or Nursing Assistant in a hospital, nursing home or home health agency. Must have successfully completed a State-established or other training and competency evaluation program that meets State and Federal requirements.
  4. Skills: Must be able to read, write, and verbally report clinical information; demonstrate interest in the welfare of the ill and elderly; proof of current CPR; successful completion of the Aide Competency Evaluation Skills Checklist and written Skills Test.
  5. Transportation: Reliable transportation with valid auto liability insurance.
Environmental and Working Conditions

Works in patients'/clients' homes in various conditions, possible exposure to blood, body fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather.

Physical and Mental Effort

Prolonged standing and walking required. Ability to lift up to 50 pounds and move patients/clients. Requires working under some stressful conditions to meet deadlines, identify patient/client needs, and make quick decisions. Requires hand-eye coordination and manual dexterity. Ability to use durable medical equipment in the home.

Essential Functions
  1. Provides direct patient/client care under the direction of the RN and according to the Aide Plan of Care.
  2. Assists patients/clients with safe transfers and ambulation per Agency policy.
  3. Reports changes and documents care provided to ensure continuity of care.
  4. Practices infection control principles; maintains a clean and safe environment.
  5. Communicates positively and respectfully with patients, families, and staff.
  6. Respects patient privacy and property.
  7. Assists with proper nutrition and fluid intake.
  8. Manages time effectively and contributes to agency operations.
  9. Performs treatments and procedures per policy.
  10. Attends 12 hours of in-services annually for professional growth.
  11. Supports the agency's philosophy and policies to ensure quality care.
