Job summary
Moments Hospice is seeking a compassionate Certified Nursing Assistant to join our on-the-road care team. You will provide personal care, support daily living activities, and help maintain patient comfort and dignity under an RN Case Manager's supervision.

Qualifications
- CNA license required and 12 months of caregiving experience.
- Current CPR certification preferred.
- Excellent customer service and strong communication abilities.
- Valid driver's license and auto insurance required.
- Ability to lift 50 lbs and apply good body mechanics.
- Experience in Home Care, Assisted Living/Memory Care, Skilled Nursing, Hospital or Hospice.
- Comfort with electronic medical records and healthcare software.
Responsibilities
- Perform personal care activities (bathing, dressing, grooming, toileting, skin care, etc.).
- Review patient care plans and assist with lifting and transferring patients.
- Assist with range of motion, positioning, exercise, and ambulation.
- Assist with dining and nutrition and maintain electronic medical records.
- Work under the direction of a Case Manager RN.
