Job summary
Moments Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to support patients' comfort and dignity in their final days. You will work under the RN Case Manager, perform personal care tasks, and help with daily living activities.
The role includes opportunities for training, a company car with fuel and insurance, a company cell phone, and comprehensive benefits. Excellent teamwork, on-call support, and sensible caseloads are emphasized.
Qualifications
- Certified Nursing Assistant (CNA) license (required)
- 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
- Current CPR certification (preferred)
Responsibilities
- Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.)
- Review patient care plans
- Assist with lifting and transferring patients
- Assist with range of motion, positioning, exercise, and ambulation
- Assist with dining and nutrition Maintain electronic medical records
- Work under the direction of a Case Manager RN
