Job summary

Traditions Health LLC is seeking a Certified Nursing Assistant (CNA) to join our Hospice Team in Decatur, IL. The CNA will provide personal healthcare to patients in their homes, ensuring a safe environment while collaborating with patients and families.

The ideal candidate will have at least one year of experience as a Home Health Aide or Nursing Assistant and a current CPR certification. Compensation ranges from $17.88 to $21.13 per hour and includes health insurance, paid time off, and a 401(k) plan.

Qualifications

• At least one year as a Home Health Aide or Nursing Assistant preferred.
• Successful completion of a state‑established training program that meets requirements.

Responsibilities

• Provide direct patient care as outlined in the Aide Plan of Care.
• Assist with safe transfers and ambulation.
• Report changes in patient condition and document care.
• Practice accepted infection‑control principles.
• Promote respectful communication with patients and families.

Skills

Excellent written and verbal communication

CPR certification (current)

Ability to lift up to 50 pounds

Reading, writing, and reporting clinical information accurately

Education

High school diploma preferred
