OptimalCare seeks a Hospice Aide/Homemaker (CNA) to deliver comfort, dignity and compassionate care to patients in their homes. You’ll assist with personal care, meal prep, mobility, and hygiene, while building trust with patients and families and working with the interdisciplinary team to support individual needs.

This full-time role involves daily travel within Jackson, MI and surrounding areas, with an 8am-5pm schedule on weekdays and weekend on-call rotation.

Qualifications

• Certified Nursing Assistant: current certification as a nurse aide in Michigan.

Responsibilities

• Provide hands-on personal care (bathing, dressing, grooming, toileting, oral care, meal prep, mobility).
• Build trusting relationships with patients and families with empathy and compassion.
• Collaborate with nurses and the care team to meet each patient’s needs.
