Job summary
Moments Hospice is seeking a compassionate CNA to join our team and help patients maintain dignity during their final days. You will perform personal care tasks, review care plans, and work under an RN Case Manager.

Qualifications
- CNA license required for this role.
- 12 months of caregiving experience in home care, assisted living/memory care, skilled nursing, hospital or hospice.
- Current CPR certification preferred.
- Excellent customer service skills and strong communication.
- Ability to work in an electronic medical record system.
- Valid driver's license and auto insurance.
Responsibilities
- Perform personal care activities (bathing, dressing, grooming, toileting, skin care).
- Review patient care plans and assist with lifting/transferring patients.
- Assist with range of motion, positioning, exercise, and ambulation.
- Assist with dining and nutrition and maintain electronic medical records.
- Work under the direction of a Case Manager RN.
