Job summary

Chapters Health System is seeking a CNA - Hospice Aide to provide personal care and homemaker assistance under RN direction, delivering compassionate support to patients and families in our communities.

Requirements include CNA certification in Florida, BLS, and reliable transportation. The role involves lifting, mobility assistance, and communicating with patients in English, with duties aligned to the Plan of Care and safety standards.

Qualifications

• High school diploma or GED or equivalent.
• Current CNA certification in the state of employment.
• Minimum six months CNA or Home Health Aide experience preferred.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• For Hope employees: BLS by January 1, 2025.
• For Capital Caring employees: BLS by November 1, 2024.
• Mobile Driver - valid driver’s license and automobile insurance per Company policy.
• Reliable transportation to meet work schedule.
• Ability to communicate (written and verbal) in English.
• Ability to manage the emotional stress of caring for ill patients and families.
• Ability to perform essential functions including lifting and moving patients/equipment.
• Must be able to access homes/facilities in assigned area (stairs, heat, no AC).
• Must be able/willing to have contact with individuals with communicable diseases.

• Under RN supervision, assist with activities of daily living.
• Report observations to RN and communicate significant issues to RN or Clinical Manager.
• Be flexible during emergencies and fulfill duties as assigned.
