Job summary

Qualifications

• Must be a Certified Nursing Assistant (CNA) in good standing.
• Experience in hospice or community health is preferred.
• Ability to follow care plans and document appropriately.

Responsibilities

• Perform personal care needs for patients per assignment and report to Case Manager if unable to complete tasks.
• Provide patient and family with respectful communication and confidentiality.
• Assist with baths, grooming, feeding, and vital signs as ordered.
• Support in mobility, transfers, and positioning; monitor safety of patient and equipment.
• Document care according to organizational procedures and care plans.
