Responsibilities

• Provide CNA-level care to home health and hospice patients in their homes or other places of residence
• Perform covered aide services according to Medicare home health and hospice guidelines
• Assist patients with activities of daily living and other personal care needs based on assessments and care plans
• Provide care under supervision of a Registered Nurse
• Develop positive relationships with patients, families, physicians and care team
• Monitor patients and report changes to the care team
• Document observations and care in a timely manner
• Use patient-care equipment safely within training and competency
