Job summary

Oaks Healthcare in Poteau, OK is seeking a compassionate Certified Nursing Assistant to support hospice patients in their homes in the evening shift. You will follow RN instructions, assist with personal care, and help maintain a safe, respectful environment.

Qualifications

• High school diploma or GED.
• CNA, CHHA, LTC licenses required.
• Driver's license and auto liability insurance.

Responsibilities

• Follows written instructions for patient care as prepared by an RN or Clinical Director in the hospice plan of care.
• Assists patients with personal care to preserve hygiene while maintaining a safe environment.
• Performs simple procedures as an extension of therapy services, such as ambulation, ROM and exercise.
• Reports changes in patient’s condition to supervisor or RN.
• Maintains record of services performed and of patient condition.
• Establishes and maintains a good working environment with patient/family and co-workers.
• Participates in patient care conferences to provide input for continuous care.
• Perform household services essential to health care at home.
