Job summary

Suncrest Hospice in Wheeling, IL is seeking a Certified Nursing Assistant (CNA) to provide compassionate resident care under the direction of the Case Manager and DON. You will work with nurses, social workers, chaplains, and a care team to deliver person-centered care aligned with assessments and care plans.

Qualifications

• CNA certification required (state(s) of practice)
• Must be at least 18 years old

Responsibilities

• Provide resident care under the direction of a Case Manager/DON as per assessments and care plans
• Coordinate care with licensed staff to ensure appropriate services
• Follow all nursing policies, procedures and safety guidelines
