Swan Hospice is seeking a Certified Nursing Assistant (CNA) with hospice experience to join our team. We provide comprehensive hospice care in skilled nursing facilities, assisted living facilities, and private homes, emphasizing kindness, respect, compassion, and dignity.

Ideal candidates hold a Michigan CNA license, have at least one year of hospice experience, reliable transportation, and a patient-centered approach to support residents and families with dignity and comfort.

Qualifications

• CNA License in Michigan
• Understands the hospice philosophy
• Minimum of one year experience working in hospice
• Dependable transportation

Responsibilities

• Assists with prescribed exercise and ROM activities as directed by Hospice Nurse
• Assists in toileting, positioning and transfer of residents
• Provides ADL support: personal hygiene, grooming, skin care, nails
• Reminds or helps patients with medication under supervision of Hospice Nurse
• Light housekeeping of patient unit/room and handling laundry
• Feeds and assists with meals
• Measures temperature, pulse, and respiration as supervised
• Reports changes to the Hospice Nurse and documents information with date/time
