Job summary

Swan Hospice is seeking a Certified Nursing Assistant with hospice experience to join our compassionate care team in Michigan. Responsibilities include assisting with ADLs, ROM, medication reminders under supervision, and supporting residents in skilled nursing facilities, assisted living, and private homes.

Ideal candidates will hold a Michigan CNA license, have at least one year of hospice experience, and dependable transportation to ensure reliable coverage across shifts.

Qualifications

• Must be a Michigan CNA with hospice experience.
• Minimum of one year experience in hospice care.
• Dependable transportation required.

Responsibilities

• Assist with prescribed exercises and ROM under supervision.
• Help with toileting, transfers, turning and positioning.
• Assist with ADLs including personal hygiene and grooming.
• Remind resident to take medications as supervised.
• Light housekeeping of resident unit and laundry as needed.
• Assist with meals and feeding as required.
• Take and report vitals (temperature, pulse, respiration) as directed.
• Report changes to Hospice Nurse and document appropriately.
