Job summary

Swan Hospice is seeking a Certified Nursing Assistant (CNA) with hospice experience to join our caring team. We serve residents in skilled nursing facilities, assisted living communities, and private homes with compassion and dignity.

The ideal candidate will hold a Michigan CNA license, have at least one year of hospice experience, and have dependable transportation. You will work under the guidance of a Hospice Nurse, supporting daily living activities and patient comfort.

Qualifications

• Hospice experience required or preferred.
• Michigan CNA license is required.
• Minimum of one year experience working in hospice.
• Reliable transportation is expected for this role.

Responsibilities

• Assists with prescribed exercises and ROM as instructed by Hospice Nurse.
• Provides assistance in toileting, positioning, transfers, turning patients.
• Assists with ADLs including personal hygiene, grooming, mouth care, skin care, nails.
• Reminds and assists patients to take medications as directed by nursing staff.
• Performs light housekeeping in patient rooms, changes linens, and handles laundry as needed.
• Feeds and assists with meals.
• Obtains temperature, pulse and respiration under supervision.
• Reports changes in patient condition to the Hospice Nurse and documents communications.
