Job summary

Transitions Hospice is seeking a dedicated CNA to provide in-home patient care across Rantoul, Champaign, Piatt and surrounding areas. Schedule is Mon-Fri with 8-hour shifts; pay range is $19.00-$24.00 per hour, plus benefits.

CNAs will join an interdisciplinary team delivering compassionate, dignified care in patients homes, addressing physical, emotional, and spiritual needs. You will monitor vitals, document care, and communicate changes to the care team with empathy and discretion, while

Qualifications

• Active CNA license in the state of employment.
• Current CPR certification.
• 1+ year Hospice, home health or related organization experience preferred.
• Comfort working with terminally ill patients and their families.

Responsibilities

• Providing personal care with dignity, comfort, and respect at the center of everything
• Attending to patient needs and requests promptly and compassionately
• Monitoring and recording vital signs (temperature, pulse, respiration, blood pressure) as directed
• Observing patients and reporting changes to the care team accurately and in a timely manner
• Communicating with patients and families in a positive, ethical, and confidential manner
• Maintaining thorough documentation in line with organizational standards
• Upholding the hospice philosophy with professionalism and genuine compassion

Skills

Active CNA license

CPR certification

Hospice/home health experience
