Presbyterian Healthcare Services is seeking a compassionate Certified Nursing Assistant to provide patient-centered hospice care in home settings across New Mexico. You will work under a registered nurse, managing daily living activities, comfort measures, and routine monitoring with a focus on dignity and quality of life.

The role involves route planning, charting in EPIC/EMR, and collaboration with the interdisciplinary team to tailor care to each patient’s needs.

Competenze

• High School diploma or equivalent.
• 1 year of direct patient care in any clinical setting preferred.
• CNA certification and BLS certification required.
• Proficiency with EPIC/EMR systems and basic MS Office.
• Valid driver’s license and reliable vehicle.

Mansioni

• Provide patient-centered care under RN direction in home hospice.
• Plan routes to patient service addresses and manage caseload.
• Record vital signs and care data in the patient chart.
• Support activities of daily living and comfort measures.
• Assist with medications prompts and follow care plans.
• Document care per agency policy and participate in IDT meetings.
