Job summary

Holisticare Hospice in Wayne, Alabama is hiring a Full Time Hospice Aide (CNA) to provide hands-on end-of-life care with our nursing team, delivering comfort, dignity and compassionate support to patients and families. You will follow a personalized plan of care, assist with daily living activities, maintain accurate documentation, and collaborate with the care team.

Requirements include a High School Diploma or GED, active CNA license, CPR certification, and reliable transportation.

Qualifications

• High School Diploma or GED is required.
• CNA certification active in the state of practice.
• Minimum 1 year CNA experience in acute or long-term care; hospice experience a plus.
• CPR certification required if state mandates.
• Valid driver’s license, vehicle insurance, and dependable transportation.

Responsibilities

• Provide personalized care following the plan of care and attending to patient and family needs.
• Assist with personal care and activities of daily living to maintain comfort and dignity.
• Demonstrate exceptional customer service and support patient satisfaction.
• Provide guidance and support to families during the patient’s end-of-life journey.
• Maintain accurate documentation in home and agency records in compliance with standards.
