Job summary

Care360 Hospice in Toledo, OH is seeking a compassionate Certified Nursing Assistant to join our hospice team. This full-time role covers 3 PM–11 PM shifts with OC rotation every 8–10 weeks and travel to patient homes in the Toledo area.

Responsibilities include patient-centered personal care, companionship, and coordination with the interdisciplinary team to ensure comfort and dignity at end of life. CPR/BLS-certified CNAs with a valid license are preferred; travel is required.

Qualifications

• Must be a Licensed Nursing Assistant (CNA) in the state of practice.
• CPR/BLS certification is required.
• Willingness to travel to patient locations.
• Hospice experience preferred; prior AL/LTC/SNF or Home Health is a plus.

Responsibilities

• Provide compassionate personal care including bathing, grooming, dressing, toileting, and feeding per the hospice aide plan of care.
• Travel to patients’ residences, including private homes and facilities, to deliver hospice services.
• Offer emotional support and companionship to patients and families to promote comfort and quality of life.
• Provide bedside vigil support during end-of-life care to ensure patient comfort.
• Assist with ambulation, transfers, positioning, and ROM as documented in the care plan.
• Safely use and assist with durable medical equipment (DME) like walkers, wheelchairs, beds, and lifts.
• Observe and report changes in patient condition or home environment to the case manager.
• Accurately document care provided per Hospice policies and procedures.
• Participate in team meetings and interdisciplinary collaboration.
• Provide support and education to family members within the CNA scope.
• Attend on-site monthly educational meetings and ongoing training.
• Perform all other duties as assigned.
