Job summary

Reserve Care (formerly Hospice of the Western Reserve) seeks a Home Care Aide to provide personal care under nurse supervision per the patient’s care plan. The role involves collaborating with a transdisciplinary team in varied settings and ensuring a safe environment.

You will assist with ADLs, transfers, ROM exercises, nutrition, basic wound care, and charting. Attendance at care conferences and on-call duties are required, with weekend shifts as needed.

Qualifications

• Must be able to read and write English proficiently.
• Must be state tested nursing assistant in good standing.
• Equivalent education/experience may substitute for minimum qualifications except when there are legal requirements such as a license/certification/registration.

Responsibilities

• Provide personal care and assist with ADLs according to the patient's plan of care.
• Provide safe environment for patients.
• Safely ambulate/transfer patient as appropriate.
• Assist with passive and active ROM exercises as directed by the nurse.
• Prepare nutritional snacks as appropriate for the patient.
• Under supervision of the nurse, assist with pre-poured medications for home care only.
• Perform basic wound, skin and bowel care per plan of care where appropriate.
• Record activities and observations in the clinical record during visits.
• Complete charting, timesheets and mileage logs as defined by agency policy.
• Attend and contribute to patient care conferences and team meetings.
• Be flexible with shifts and weekends as needed.
• Participate in on-call rotation (Home Care only).
• Provide caregiver education on turning/positioning and transfers.
