Embrace Hospice in Kerrville, TX is seeking a dedicated Hospice CNA to join our team. The role focuses on providing compassionate 1:1 personal care, monitoring patient needs, and supporting families in a comforting home setting.

You will work within a supportive interdisciplinary team, with a schedule that offers stability and opportunities for growth. CPR certification and state training requirements are required for this position.

Qualifikationen

• Minimum six months of personal care experience.
• Must be 18+ years old.
• CPR certification required.
• Complete a minimum of 75 hours of classroom and supervised training, with 16 hours of practical training.

Aufgaben

• Provide 1:1 personal care including bathing, dressing, toileting, mobility and repositioning.
• Monitor patient condition and report changes to the team.
• Assist with ambulation, transfers, ROM exercises and safe equipment use.
• Support medication administration as allowed by state regulations.
• Offer emotional support and companionship to patients and families.
• Light housekeeping and meal assistance; maintain a clean space.
• Maintain accurate and timely documentation of care.
• Collaborate with hospice interdisciplinary team on care plans.
