Job summary

Embrace Hospice in Kerrville, TX is seeking a compassionate CNA/Hospice Aide to provide 1:1 patient care in home settings within Kerrville and surrounding areas.

You will assist with personal care, monitor changes, document care, and collaborate with an interdisciplinary team to support patients and families. Reliable, CPR-certified CNAs with good communication and organizational skills are encouraged to apply.

Qualifications

• Minimum six months of care experience.
• 18+ years old.
• Complete a minimum of 75 hours of classroom and supervised practical training, with 16 hours prior to mandatory 16 hours of practical training.
• Possess and maintain current CPR certification.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and comfortable providing specialized care to the terminally ill.
• Ability to work with little direct supervision.
• Ability to handle death/dying.
• Strong oral and written communication skills.
• Good organizational skills.

Responsibilities

• Provide 1:1 patient care to make a lasting impact on patients and families.
• Monitor and report changes in the patient's condition.
• Assist with ambulation, transfers, ROM exercises and safe equipment use.
• Help with self-administered medications per state regulations.
• Offer companionship and comfort to patients and families.
• Change linens and assist with meals and maintain a clean space.
• Maintain accurate and timely documentation.
• Collaborate with hospice interdisciplinary team.
