Job summary

Adoration Health is seeking a compassionate Hospice CNA/STNA to join our team in Xenia, OH. You will provide 1:1 care, monitor patient conditions, and support families during a critical time in the patient’s life.

Qualifications

• Minimum six months experience providing personal care.
• Must be 18+ years of age.
• Complete a minimum of 75 hours of classroom and supervised practical training, including 16 hours prior to a mandatory 16 hours of practical training.
• Possess and maintain CPR certification.

Responsibilities

• Provide 1:1 personal care, including bathing, dressing, toileting, mobility and repositioning.
• Monitor and report changes in patient condition.
• Assist with ambulation, transfers, ROM and safe equipment use.
• Help with self-administered medications per state regulations.
• Offer emotional support and companionship to patients and families.
• Perform light housekeeping and meal assistance.
• Maintain accurate and timely documentation.
• Collaborate with hospice interdisciplinary team.
