Job summary

Elara Caring is seeking a Clinical Team Manager in Michigan to lead hospice patient care services and supervise the care team, ensuring regulatory compliance and high-quality outcomes. The role focuses on coordinating home health services, staff scheduling, and care coordination to meet patient needs.

You will partner with the Branch Director to monitor performance metrics, coach staff, and guide interview/hiring processes while maintaining a patient-first approach in a fast-paced home health

Qualifications

• Associate degree in nursing or related field required.
• Current, unrestricted RN license valid for the state of work.
• Supervisory experience in home care or hospice preferred.

Responsibilities

• Oversee patient care services, staff assignments, referrals, and care coordination.
• Plan and coordinate Home Health services with patients, families, physicians, and facilities.
• Ensure timely start of care and regulatory compliance.
• Review and approve plans of care and physician orders.
• Conduct regular OASIS reviews and ensure quality patient care.
• Coach and support direct care staff and assist with hiring.
• Monitor performance metrics and support clinical operations.

Hospice Clinical Team Manager

$5,000 Sign-On Bonus

Responsibilities

• Oversee patient care services, staff assignments, referrals, and care coordination to ensure patient needs are met.
• Plan and coordinate Home Health services with patients, families, physicians, providers, vendors, and facilities.
• Ensure timely start of care and ongoing compliance with applicable regulatory guidelines.
• Review and approve individualized Plans of Care, physician orders, and proposed changes for clinical appropriateness.
• Conduct regular OASIS reviews to support accurate documentation and quality patient care.
• Provide ongoing guidance, coaching, and support to direct care team members and assist with interviewing and hiring clinical staff.
• Partner with the Branch Director or Administrator to monitor key performance metrics and support clinical operations, including field work as needed.

Requirements

• Associate degree in a Nursing related field, Bachelor preferred
• 2 years of home care experience in Home Health or Hospice environment
• 1 year of supervisory experience required
• Current, unrestricted RN license valid for the state of work
• Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
