Bristol Hospice is seeking a Clinical Supervisor in Green Bay, WI. The role is full-time, day shift, supervising clinical staff and ensuring highest quality hospice services within the Green Bay office.

Qualifications

• Active RN license in good standing.
• Experience in hospice/home care with two years of management or supervisory experience.
• Strong communication and interpersonal skills.

Responsibilities

• Receive case referrals and review patient information to determine hospice needs.
• Assign appropriate hospice personnel to referral cases as needed.
• Coordinate with Medical Director and Attending Physician on eligibility for services.
• Review cases via home visits, conferences, records review and clinical services.
• Assess patient diagnoses, prognosis, medications and clinical course.
• Guide clinicians to improve performance and quality of care.
