Job summary

Elara Caring is seeking a Clinical Success Mentor - Hospice to join our team in Oklahoma City. You will lead hospice education, ensure regulatory compliance, and coach interdisciplinary teams to improve patient outcomes.

The role requires an RN with 3–5 years in hospice, a valid driver’s license, and willingness to travel 50–75% between patient homes, reporting to the Director of Clinical Success Mentorship.

Qualifications

• Bachelor's degree in nursing or higher is required.
• Current RN license with compact licensure preferred.
• Minimum 3–5 years' hospice nursing experience with leadership exposure.

Responsibilities

• Maintain Medicare/Medicaid and hospice regulatory knowledge.
• Coach on eligibility criteria, standards, and documentation guidelines.
• Educate staff and physicians on hospice requirements and compliance.
• Train IDG/IDT on roles, collaboration, and regulatory practices.
• Lead ongoing education on regulatory changes and audits.

As a Clinical Success Mentor - Hospice you'll contribute to our success in the following ways:

• Maintains current knowledge of Medicare/Medicaid, hospice, third-party payor regulations, and applicable federal, state, and local requirements.
• Provides training and coaching on hospice eligibility criteria, regulatory standards, and disease-specific guidelines.
• Educates clinical staff and physicians on medical, functional, prognostic, and documentation requirements supporting hospice eligibility.
• Trains and supports interdisciplinary teams (IDG/IDT) on roles, responsibilities, collaboration, documentation, and regulatory compliance.
• Conducts ongoing education on regulatory changes, quality standards, audits, and hospice best practices.
• Assesses clinical quality across nursing, social work, and spiritual care through IDG observation, audits, and eligibility scoring.
• Identifies opportunities for improvement and provides coaching, training, resources, and follow-up to strengthen clinical quality and compliance.
• Attends IDG meetings across branches to assess processes and provide guidance to improve quality and compliance.
• Supports key organizational initiatives, including live discharges, eligibility and documentation, and IDG process improvement.

What is Required?

• Bachelor's degree or Graduate of an accredited program of Nursing.
• Current State License as a Registered Nurse, with a Compact Licensure preferred.
• Minimum of three to five (5) years' relevant experience as a registered nurse in a Hospice environment with quality, compliance and/or leadership experience preferred.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• TRAVEL: 50-75% travel.
• Meets all applicable agency policies and procedures related to health screening and required testing.

You will report to the Director of Clinical Success Mentorship.
