Job summary

Pathways Home Health & Hospice in Sunnyvale is seeking a Manager Clinical Services (MCS) to coordinate clinical care, ensure documentation quality, and act as a clinical resource. The role involves staff supervision, policy adherence, and collaboration with physicians and multi-disciplinary teams.

The position requires an RN license (BSN preferred) or Doctorate of Physical Therapy, with at least two years of home health nursing experience and strong leadership capabilities.

Qualifications

• Current Registered Nurse (RN) license (BSN preferred) in good standing to practice in the state(s) is required or Doctorate of Physical Therapy.
• Two years homecare nursing experience is required.
• Excellent clinical skills.
• Thorough knowledge and understanding of home care regulatory requirements.
• Strong written and verbal skills.
• Ability to work closely with a wide range of professionals or a multi-disciplinary team.
• Demonstrated leadership, administrative and supervisory skills.
• Excellent organizational and time management skills.
• Excellent attention to detail is required.

Responsibilities

• Adheres to the Corporate Compliance Program, including confidentiality of HIPAA protected health information.
• Responsible for processing assigned workflow in a timely accurate manner and supporting office and clinical staff.
• Enters and approves orders for tracking purposes and ensures they are sent to physician for signature.
• Reviews and processes vital sign alert reports and documents action and follow-up.
• Supports and promotes department and organizational decisions and collaborates with team.
• Creates and manages performance of direct reports and coaches staff for excellence.
• Ensures appropriate staffing levels and participates in interviews and onboarding.
