Qualifications

Acts is currently seeking qualified candidates for our Hospice Clinical Manager position.

In this role, you will oversee and supervise the implementation of an interdisciplinary, individualized plan of care for each participant and family/caregiver unit, including assessment and management of pain and undesirable symptoms which affect comfort. You will also serve as a liaison between the Interdisciplinary Team (IDT), the Medical Director, the Hospice Clinical Director and Hospice Administrator regarding clinical problems and progress in care of hospice participants. The Hospice RN Care Manager will directly supervise hospice nursing personnel.

Requirements

The ideal candidate will meet the following requirements:

  • Current state Registered Nurse (RN) license
  • Minimum of 2-3 years experience in hospice or a transferable, clinical venue such as home care, geriatrics, or oncology
  • Current or eligible for certification in CPR
  • Certification in hospice and palliative nursing within two years of employment is strongly encouraged

Pay Range
$70,555.00 - $88,816.00 / year. Starting rate will vary based on skills and experience.
