Qualifications

• Registered nurse with current licensure to practice in the state.
• BSN degree preferred or 1 year nursing experience in the last 3 years.
• 2–3 years hospice/home health or community health with supervisory experience.
• Adheres to professional standards and practices.

Responsibilities

• Receive case referrals and assign hospice personnel to cases.
• Coordinate care through home visits, conferences, and record review.
• Guide clinicians to improve performance and service delivery.
• Assist in planning goals, priorities, and care plans.
• Lead case conferences for interdisciplinary coordination.
• Participate in quality improvement teams and education programs.
• Assist in interviewing and orienting new staff.
• Support policy development and organizational goals.
