Job summary

Elara Caring in Saginaw, MI is seeking a Hospice Clinical Team Manager to lead the clinical team, oversee patient care services, and coordinate referrals and care plans to meet patient needs in home health and hospice settings.

You will supervise staff, participate in hiring, ensure timely start of care, conduct OASIS reviews, mentor clinicians, and work with leadership to monitor KPIs; applicants must have an RN license and relevant experience.

Qualifications

• RN license valid in state of work (Michigan).
• Associate degree in Nursing or higher; Bachelor’s degree preferred.
• At least 2 years of home health or hospice experience.
• Minimum 1 year of supervisory experience.
• Ability to use Homecare Homebase, Workday, and Microsoft Office.

Responsibilities

• Oversee patient care services, staff assignments, referrals, and care coordination.
• Plan and coordinate Home Health services with patients, families, physicians, providers, vendors and facilities.
• Ensure timely start of care and compliance with regulatory guidelines.
• Review and approve individualized Plans of Care and physician orders.
• Conduct regular OASIS reviews to support accurate documentation.
• Provide guidance and coaching to direct care team; assist with interviewing and hiring.
• Partner with Branch Director to monitor KPIs and support clinical operations.

Hospice Clinical Team Manager

Saginaw

As a Clinical Team Manager , you’ll contribute to our success in the following ways:

• Oversee patient care services, staff assignments, referrals, and care coordination to ensure patient needs are met.
• Plan and coordinate Home Health services with patients, families, physicians, providers, vendors, and facilities.
• Ensure timely start of care and ongoing compliance with applicable regulatory guidelines.
• Review and approve individualized Plans of Care, physician orders, and proposed changes for clinical appropriateness.
• Conduct regular OASIS reviews to support accurate documentation and quality patient care.
• Provide ongoing guidance, coaching, and support to direct care team members and assist with interviewing and hiring clinical staff.
• Partner with the Branch Director or Administrator to monitor key performance metrics and support clinical operations, including field work as needed.

What is Required?

• Associate degree in a Nursing related field, Bachelor preferred
• 2 years of home care experience in Home Health or Hospice environment
• 1 year of supervisory experience required
• Current, unrestricted RN license valid for the state of work
• Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
